Each Person Needs to Bring one of the following Forms of ID:

#2 HAVE YOU BEEN PREVIOUSLY MARRIED?

If ANY of these events took place in the last 2 years you MUST bring proof or we will not be able to execute the Marriage License. EVEN if the event happened over 2 years ago you will still need to know the Day, Month and Year of your last marriage ending:

Divorce

Death Certificate

Annulment

Domestic Partnership

#3 Birth parents full names

We need to go off of your birth certificate. Whomever was named on your birth certificate for parents, you will need to know their full maiden (Name before marriage) (First, Middle and Last).

If a parent was not listed on your birth certificate we are allowed to put UNKNOWN

#4 Must put the same home address

You must tell us you live at the same address.
